Updated: 10OCT2008 Florida Department Of Transportation Notice To Contractors / Consultants: The Department of Transportation is soliciting contracting and consulting services for the design/build projects identified below. Qualified consultants/contractors are encouraged to submit a letter of interest, for each project in which they are interested, to the indicated requesting unit by 5:00 PM Eastern time on the date indicated as the Response Deadline. CONSULTANT ELIGIBILITY: It is a basic tenet of the Depart- ment's contracting program that contracts are procured in a fair, open, and competitive manner. The Department requires that consultants representing the Department must be free of conflict- ing professional or personal interests. In order to prevent potential conflicts of interest, the Department has established guidelines to be followed by professional services consultants. Please familiarize yourself with the requirements of Directive no. 375-030-006, also known as: "Restriction on Consultants Eligibility to Compete for Department Contracts". By submitting a Letter of Interest or bid proposal as part of a design-build team, the design professional certifies that they are in compliance with Directive No. 375-030-006. This directive is available at the following link: Professional Services Publications Menu For those projects designated as ADJUSTED SCORE TYPE, letters of interest are requested by the end of business on the date indicated below. The letters of interest will be limited to the page limit indicated below and will clearly indicate a name, address, phone number, and e-mail address for a contact person; the key companies and personnel proposed as partici- pating in the project; and the prequalification status of the contractor and professional services team members. The Department will judge the relative ability of each submitting company to perform the required services based on pre- qualification information, past performance with the Department, and the letter of interest. At least three teams will be shortlisted from the received responses at the date specified below and the results of the shortlisting process will be posted on the Department's web site (www.dot.state.fl.us/procurement) at the indicated date and time. Public meeting dates and times are provide with new listed projects. All public meetings will be held in District head- quarters unless otherwise noted. Changes to meeting dates and times will be updated under the Planned Projects link. The short listed firms will be provided a Request for Proposal(RFP) contain- ing a design criteria package and requested to provide a technical proposal and price bid proposal for the project. The award of the project will be based on an adjusted score based on both the price and technical proposal score. For those projects designated as LOW BID TYPE, bid proposals are requested by close of business on the date indicated in the advertisement. For these type projects, an RFP may be requested by any interested party at the response address provided below. The technical proposal of the low bidder will then be reviewed to determine its responsiveness. If found to be non-responsive, the next lowest bidder will be con- sidered. A proposal will be considered non-responsive if it does not contain all of the information and level of detail requested in the RFP. All bid openings and public meetings will be held in district headquarters, unless otherwise noted. Changes to meeting dates and times will be updated under the Planned Projects link. The selection results will be posted on the Department's web site(www.dot.state.fl.us/procurement) at the indicated date and time. Note: The Department reserves the right to reject all proposals and/or to waive minor proposal irregularities. Pursuant to Sections 120.57(3) and 337.11, Florida Statutes, and Rule Chapter 28-110, Florida Administrative Code, any person adversely affected by the agency decision or intended decision shall file with the agency both a notice of protest in writing and bond within 72 hours after the posting of the notice of decision or intended decision, or posting of the solicitation with respect to a protest of the terms, conditions, and specifications contained in a solicitation, and will file a formal written protest within ten days after the filing of the notice of protest. The required notice of protest and bond, and formal written protest must each be timely filed with the Florida Department of Transportation, Clerk of Agency Proceedings, 605 Suwannee St., Mail Station 58, Room 550, Tallahassee, FL 32399-0458. Failure to file a protest within the time prescribed in section 120.57(3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under chapter 120, Florida Statutes. ************************************************************ DISTRICT 2 Procurement Number : E2M37 Project Description : PROJECT NAME: The Florida Department of Transportation (Department) has issued this Request for Proposal (RFP) to solicit competitive bids and technical proposals from Proposers for landscape design and installation of median landscaping for SR 15from Gum Street to Lamont Street (0.937 miles) and SR 21 from Halpren's Way to CR 220 (1.396 miles) in Clay County. DESCRIPTION OF WORK: On Project 20820235201 - SR 15 (N. Orange Ave.) from Gum St. to Lamont St., Clay County, the work shall consist of removal of existing landscaping, design and implement a new landscape plan throughout the center of five (5) curbed medians which includes cabbage palms, crepe myrtles, shrubs, ornamental grass and perennial peanut sod as well as flower bed preparation on each median nose. Project includes design and construction of a permanent irrigation system per each curbed median. Finally, maintenance including mowing of the curbed medians will be required of the contractor during the contract and throughout the one (1) year establishment period. Right-Of-Way outside the curbed medians will not be part of this contract. . On Project 20821165201 - SR 21 (Blanding Blvd.) from Halpren's Way to CR 220, Clay County, the work shall consist of removal of existing landscaping, design and implement a new landscape design with crape myrtles planted throughout the center of nineteen (19) curbed medians at the specified spacing. Also, maintenance including mowing of all the medians within project limits will be required of the contractor during the contract and throughout the one (1) year establishment period. Right-Of-Way outside the roadway medians will not be part of this contract. The Design-Build Firm shall be responsible for survey, geotechnical investigation, design, acquisition of all permits not acquired by the Department, any required modification of permits acquired by the Department, maintenance of traffic, demolition, and construction on or before the date indicated in their proposal. The Design-Build Firm will coordinate all utility relocations. . The Design and Construction Criteria (Section VI) sets forth requirements regarding survey, design, construction, maintenance of traffic during construction, requirements relative to project management, scheduling and coordination with other agencies and entities such as state and local government, utilities, environmental permitting agencies and the public. . The Design-Build Firm shall demonstrate good project management practices while working on this project. These include communication with the Department and the District Landscape Architect and others as necessary, documentation, management of time and resources. . There will be a one (1) year establishment period following installation. During this time period the Design-Build firm will be responsible for planting survival, project maintenance, and mowing as stated in FDOT Standard Specification Section 580. . DESIGN BUILD TEAM REQUIREMENTS: The Contractor and Design professionals cannot be on more than one team. The secondary member (i.e., designed or contractor) of the Design-Build team cannot change, after award, without the written approival of the District Secretary. A professional firm shall not submit a proposal for CEI services, either as a Prime or a Sub, for a Design-Build contract for which the same firm or its affiliate is the Engineer of Record (EOR) or is Sub to the EOR. . A consultant firm, its affiliate, or subconsultant that is under contract with the Department to develop the Request for Proposal for a Design-Build contract cannot be part of a Design-Build Team proposing on that contract as a prime or a subconsultant. A consultant firm, its affiliate, or subconsultant, that is under contract with the Department to provide Construction Engineering and Inspection (CEI) services on the Design-Build contract cannot be part of a Design-Build Team proposing on that contract as a prime or subconsultant. . PREQUALIFICATION: Pre-qualification requirements for Design-Build Advertisements for the contractor and design professional members of the Design-Build team must be in accordance with the following (Rule Chapter 14-91 F.A.C.) (1) Contractors submitting as lead or primary party with the Design-Build team must be qualified in the advertised construction contractor work classes, in accordance with the provisions of Rule Chapter 14-22, F.A.C. In this case, where the design professional is not the lead of the Design-Build team, the design professional will be utilized to meet the professional consultant work type requirements, in accordance with provisions of Rule 14-75, F.A.C. (2) Design professional sumbitting as lead of the Design- Build team may utilize subcontractors to meet the advertised construction contractor work class requirements, in accordance with the provisions of Rule Chapter 14-22, F.A.C. In this case, the lead design professional will meet the professional consultant work type requirements, in accordance with provisions of Rule 14-75, F.A.C. (3) When a joint venture party submits a Letter of Interest, one of the contractor members of the joint venture party must be assigned to meet the advertised construction contractor work class requirements, in accordance with the provisions of Rule Chapter 14-22, F.A.C. In this case, the design professional member of the team will be utilized to meet the professional consultant work type requirements, in accordance with provisions of Rule 14-75, F.A.C. RESPONSE PROCEDURE: Prequqlified firms interested in being considered for the project should request a project package (Project Criteria and Request for Proposal) by submitting the fax request form found on the District Two District Contracts website located at: http://www.dot.state.fl.us/contractsadministrationdistrict2. SELECTION PROCEDURE: Firms shall submit a bid proposal and a technical proposal. All bids will be opened and the technical proposal of the lowest bidder shall be evaluated by the FDOT to determine if it is responsive and responsible. If the proposal is deemed non-responsible or non-responsible, the technical proposal of the next lowest bidder will be evaluated. The Department shall reserve the right to reject any and all proposals and waive minor proposal irregularities. PROTEST RIGHTS: "Failure to file a protest within the time prescribed in section 120.57(3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under chapter 120, Florida Statutes."
